The Freedom Commons Wellness Center is located in Cumming for easy access. We’re committed to providing you with affordable healthcare. Our dedicated healthcare professionals are ready to serve as your primary care provider or provide expert care when you feel unwell.
Our services are available at no cost to employees and dependents enrolled in the Forsyth Health Plan. A $15 visit fee applies for non-plan employees.
Our Services include: primary care, acute/sick care, chronic disease management, lab work, medication refills, sports physicals, referrals, and annual wellness/preventative exams.

Your medical information is completely private. All patient records are protected under HIPAA (Health Insurance Portability and Accountability Act). Only the clinical care team has access to your records to provide appropriate care.
